How it works
From first chat to first visit
Seven simple steps, taken at your pace. You can stop and ask questions at any point.
Enquiry
You, a family member or a professional gets in touch. We'll have an informal chat about what you're looking for, with no pressure.
Assessment
We visit you at home to get to know you: your routines, what you enjoy, what you find harder and what you'd like to achieve.
Setting expectations
We talk openly about what we can and can't do, when we'll visit and how we'll keep in touch, so there are no surprises.
Risk assessment
Together we look at anything that might affect your safety at home or out in the community, and agree sensible ways to manage it.
Your support plan
We write a plan with you, in plain English, setting out how you want to be supported. You'll get a copy.
Agreement
We put everything in a clear written agreement covering hours, fees and boundaries, signed by you or your authorised representative.
Support begins
We start, and we keep checking in. Your plan is reviewed at least once a year, or sooner if anything changes.
Ways to arrange support
However your support is paid for, the process is the same. You can come to us through:
- Your local authority
- Direct Payments
- Personal health budgets
- Paying privately (self-funding)
- Family arrangements
- Recommendation from someone you know
If something isn't right
We want to hear about it, big or small. If you raise a concern, the partner who isn't involved will look into it, keep a record and get back to you. We note small "niggles" too, because they help us get better.
Keeping people safe
If you're ever worried someone is in immediate danger, call 999. Kerry and Sami will both act as safeguarding leads for Kersami Care.
